Generic AI documentation is not sufficient in everyday clinical practice

Many systems deliver fast but poorly customizable documentation. AIRAdoc supports structured, department-specific texts according to your specifications, so that documentation is not only faster, but also more precise, controllable, and consistent.

Raw Transcript

“The patient came to the clinic today with shortness of breath. She is using the inhalers we discussed last time. Her vitamin B12 level was extremely low at 172 pg/ml. We discussed the ganglion on her wrist. She wants further evaluation of her breathing issues and nighttime cramps...”

Structured note

HISTORY

Follow-up for respiratory symptoms, Vitamin B12 deficiency, and ganglion cyst evaluation. The patient reports breathing difficulties and nocturnal muscle cramps.

FINDINGS

Vitamin B12: 172 pg/ml (significantly decreased).

ASSESSMENT / PLAN

Vitamin B12 deficiency (D51.9) — initiate supplementation. COPD (J44.9) — continue current inhalation regimen.

Documentation according to your specifications

  • Speak naturally

Record in-person or video calls; pause and resume the conversations at any time; add more content after the call if needed.

  • Generate from your templates

Not based on a one-size-fits-all template.

  • Edit securely

Rich text editing, version history, and one-click restore.

  • Share with your care team

    Organization-level visibility, when you decide.

  • Export your way

As plain text, HTML, or in custom formatting for EHR systems.

  • Capture conversations in multiple languages

The user interface and template library support English and German versions, regardless of the conversation language.

From visit to finished note in four steps

  1. Start a visit

Select a template according to the type of document you want to create. You can convert a note into another template at any time later.

  2. Capture the encounter

Voice activity detection ensures that the recording remains focused on speech. Video visit mode records system audio for telemedicine. Lab values or contextual information can be added in the additional comments.

  3. AI structures your documentation

The template you choose determines the output. A medical content filter flags informal or non-clinical recordings instead of converting them into the note format.

  4. Review, edit, export

Refine the text in the integrated editor, save explicit versions if needed, and then save and copy it in the format your EHR system expects.

Fully customizable – four ways to document

Every practice documents differently. AI Documentation supports four template architectures.

  • Custom notes

Routine patient encounter notes generated from predefined clinical elements – history, exam, results, medications, assessment/plan, and more.

  • Reports

Radiology reports, diagnostic summaries, imaging results, and laboratory findings. Practice using your own sample documents.

  • Procedures

Surgical reports and procedure documentations with example-oriented formatting that matches your team's writing style.

  • Letters

Communication with colleagues, patients, insurers, and specialists – in your chosen voice and tone.

Drag sections into your workflow

Create custom note templates from an extensive library of elements. Choose from subjective information, exam reports, results, medications, assessments/plans, and dozens of specialty-specific blocks – and then rearrange them using drag-and-drop.

  • Searches the element library
  • Drafts sample documents
  • Fetches templates from the Marketplace
  • Converts existing notes

AIRAdoc Scribe is part of a comprehensive suite of AI tools that enhances every step of patient care and maximizes practice efficiency.

Designed for clinical reality

The documentation is not yet complete when the AI generates a note.

  • Full rich-text editor for the generated note
  • Transcript review – check and edit source material
  • Additional comments – include laboratory results, reports, or contextual information for the analysis
  • Reprocessing – regenerate after transcript edits or template changes
  • Resume visit – return after the visit and add further dictations
  • Transfer content – create a new document without overwriting the original
  • Change the template or the pronoun, if your workflow requires this.

Every change is logged. Every version can be restored.

Medical documentation requires an audit trail. AI-assisted documentation includes comprehensive version control.

Share documentation with your care team

The AI documentation supports collaborative workflows for drafting and reviewing, such as a resident starting a note or discharge summary and an attending physician finalizing it before the clinical team copies the final version into the electronic health record.

Shared documents – a discharge summary draft with the steps Resident created draft, Shared with organization, and Final version ready
  • Share with organization
  • Supporting workflows between residents and senior physicians
  • Encryption-aware sharing
  • Cancel sharing at any time
  • Filter views

AIRAdoc supports teams in collaboratively creating the draft documentation. Your electronic health record (EHR) remains the system of record; after reviewing the entry, the care team copies the final text into the EHR.

Your format. Your workflow.

Once the note is complete, transfer it seamlessly into your data entry system.
